Register a Student

Welcome to the District

Registration for new students is held at the High School Main Office located at 14 Spring Street, Schuylerville, NY 12871. Normal Business Hours are 8 a.m. – 4 p.m. Hours will vary during summer and school vacation periods.

The first step in new student registration is to complete the required documents below, one set of paperwork is required for each student. Once these forms are complete and you have all additional forms required, please call the Registrar Office at 518-695-3255, ext 2237 to schedule an appointment or if you have any questions regarding registration. Registration forms can also be obtained in the attendance lobby of the High School if you are unable to access at home.

Proof of Residence

Bring a minimum of two proofs of residence to your appointment. Must contain parent/guardian name, district address dated within 30 days.

  • Gas/Electric Bill
  • Homeowner’s Insurance Policy
  • Lease
  • Renter’s Insurance
  • Cable TV Bill
  • School Tax Bill
  • Mortgage Statement
  • Auto Insurance
  • Voter Registration
  • Pay Stub (current)

Health Office Requirements

  • Immunization Record
  • Doctor’s Physical
  • Health Registration Form (see above)

Proof of Student Birth

  • Birth Certificate
  • Valid Passport or Visa (if applicable)

Proof of Parental Relationship

Must present at least one grouping:

  • Student Birth Certificate AND parent photo ID
  • Court custody documents (if applicable) AND custodian’s photo ID
  • Guardianship documents (if applicable) AND guardian photo ID
