Digital equity survey

Dear Schuylerville families,

For the second year in a row, the NYS Education Department is requiring  parents/guardians of a K-5 student to complete a Digital Equity Survey (if parents have multiple K-5 students, a survey must be completed for each student). Collecting accurate data regarding digital resource access for New York students will greatly help educators to better serve their students and families. This survey will also provide information on student access to devices and internet access in their places of residence.

Please note: middle school and high school students (grades 6-12) are completing the digital equity surveys through SchoolTool as a student user during the school day. The elementary survey (grades K-5) is a Google Form to be filled out by parents.
