Schuylerville students interested in participating in a fall sport should sign up now through Family ID by filling out the Intent to Participate Form. Please be advised that this form is just the first step in the registration process and not the final registration form. Those who have completed the Intent to Participate Form will be notified once the final registration period opens.

Student-athletes must also have a current health physical (that is valid for the entire upcoming season) on file prior to the start of the season. Student-athletes in need of an updated physical will need to either provide the nurse with a copy of a physical completed by their primary care provider or request via the option on this form that the nurse schedule a physical with the school physician or nurse practitioner. Physicals will be offered at the high school on Wednesday, May 22 from 8 to 10 a.m. Physicals will be offered at the middle school on Wednesday, June 12 from 8 to 10 a.m.
