Please be advised that due to new regulations, any child entering school for the first time, or entering kindergarten, first, third, fifth, seventh, ninth and eleventh grades must have a physical examination within 12 months of school entrance. The new statewide physical form is available by clicking here. Please drop off the completed form to the nurse in your child’s school building, or fax to the health office numbers below.
