Upon entering the seventh
grade, Schuylerville students are eligible to
participate in interscholastic athletics.
Sports Registration
If your child would like to play a sport, a registration
form for that sport must be completed and returned to
the school. These forms are available in the main
office.
Pre-Participation Physical Examination Questionnaire
If your child is considering participating on an
athletic team, a Pre-participation Physical Examination
Questionnaire must be completed. Follow

Physical Examination
Any child who plans to participate must have a physical
examination before the start of the sport season. The
district will provide an athletic physical by our school
doctor or parents may choose to have a physical
completed by their family physician. Please download,
complete and return the
Health Certificate/Appraisal Form

Health History Re-evaluation
If you've previously played an interscholastic sport and
have received a complete medical examination you'll
still need to complete a Health History Re-evaluation prior to
the start of each sport season, before practice or
try-out session begins. The state regulations mandate
that a review of each athlete's health history be
conducted. The Health History Re-evaluation form must be
returned to the health office before clearance for
athletic participation will be given. Athletic
participation will be denied until the form is
completed, signed, and returned to the health office.
