Step-by-step instructions
As a parent of an eighth grade student, the athletic department would like to make the sports sign-up process easier for you and your child. Any student participating in interscholastic athletics must have the following forms on file in the nurse’s office:
- A Student Physical Exam Form completed by a physician.
- A Pre-participation/Interval Athletic Health History Form completed by a parent/guardian.
Following the procedures below will ensure your son/daughter will be prepared for the Aug. 14 starting day of high school interscholastic sports.
Step-by-step process for fall sports sign-ups
1. On April 24 and April 25, any student interested in fall sports will be sent during specials to the school nurse to sign up. Note: students signing up after April 25 should report to the nurse’s office prior to May 24.
2. Have your son/daughter get the required school physical by the school physician*. These physicals will take place at no cost to the parents during the following dates: May 24, May 31, June 14, July 26 and Aug. 9. Sign up for a physical by clicking here. NOTE: ALL athletes wishing to schedule a physical should sign up under high school, regardless of grade.
3. Download or pick up a Pre-participation/Interval Athletic Health Form and return it to the high school nurse’s office anytime between July 14 and Aug. 7. Note: the Pre-participation/Interval Athletic Health Form must be reviewed and approved by the school physician for participation.
*Parents who wish to use a family physician must use the Student Physical Exam Form. The student is not cleared for sports participation without the district’s school physician’s review and clearance.
